HOW TO OBTAIN A LIBRARY CARD
CARDS ARE ISSUED, RENEWED, OR REPLACED ANY TIME BETWEEN THE HOUR THE LIBRARY OPENS UNTIL 15 MINUTES BEFORE CLOSING.


RESIDENTS OF HARWOOD HEIGHTS AND NORRIDGE:
1. Bring 1 proof of residency to the Library Services desk. The ID must include your name and current address.

FORMS OF ACCEPTABLE ID:

Drivers license
State ID
Bank check
Utility bill
Voters card
Mortgage statement
Lease agreement
Piece of current mail
School ID with home address
Tax bill
2. Fill out an application.

RESIDENTS UNDER 14 YEARS OF AGE:
1. Bring 1 proof of residency to the Library Services desk. May use ID with the family name and address. Forms of acceptable ID
2. Fill out an application. A parent's or guardian's signature will be required.

RESIDENTS OF CHICAGO:
1. Bring a photo ID AND your current, valid Chicago library card to the Library Services desk.
2. Fill out an application.

RESIDENTS OF OTHER LIBRARY SYSTEMS:
If your library participates in reciprocal borrowing with our library, you may use your card here.

1. Bring your current, valid library card and 1 ID to the Library Services desk. The ID must include your name and current address. Forms of acceptable ID
2. Fill out an application.

RESIDENTS OF UNINCORPORATED AREAS, SUCH AS NORWOOD PARK TOWNSHIP:
You must apply for a library card at the closest library to your residence.
You will be required to pay a non-resident fee.

If Eisenhower Library is your closest library:
1. Bring one piece of ID to the Library Services desk. The ID must include your name and current address. You will also need to bring a copy of your second installment Illinois property tax bill. The non-resident fee is determined as a percentage of your property tax. Non-resident cards are valid for one year, and every member of the household is eligible for their own card.

LIBRARY CARD RENEWAL:
Residents of Harwood Heights and Norridge:
Bring your card and 1 form of ID with your name and address to the Library Services desk. Forms of acceptable ID. Fill out an application.
Residents under 14 years of age bring your card and 1 ID with family name and address.
Residents of Chicago:
Bring your current, valid Chicago card, your MLS system card, and a photo ID with your name and address to the Library Services desk. Fill out an application.
Residents of unincorporated areas, such as Norwood Park Township:
Bring your library card, a copy of your second installment Illinois property tax bill and 1 ID with your name and address to the Library Services desk. Forms of acceptable ID. Fill out an application. Pay the current non-resident fee.

TO REPLACE A CARD:
Residents of Harwood Heights and Norridge:
Bring 1 form of ID with your name and address to the Library Services desk. Forms of acceptable ID. Fill out an application. Pay a $5.00 replacement fee.
Residents of Chicago:
Bring your current, valid Chicago card and photo ID with your name and address to the Library Services desk. Fill out an application. Pay a $5.00 replacement fee.
Residents of unincorporated areas, such as Norwood Park Township:
Bring 1 form of ID with your name and address to the Library Services desk. Forms of acceptable ID. Fill out an application. Pay a $5.00 replacement fee.
